    try getting the form from medicare office itself.

    if your physician doesn’t have the form try going to the hospital and asking to speak to the diabetes educator. Every hospital should have someone who will be able to give you either the forms or at least the information that you will need.

    Have to have the doc that handles your diabetes fill it out. He has to certify all sorts of stuff….. I deal with those ALLLLL day long….

    you can get the form at walgreens, and if they fill your prescriptions, they will handle all of that paperwork for you. i USED TO work for them, so this is not an advertisement.

    Online? or, ask for a letter, or the form from your doctor?

    Pharmacies have them,we fill them out everyday,call to double check..some pharmacies do not accept coinsignment (kmart didnt)…Does your doctors office have a pharmacy? You need to get your supplies from that pharmacy,you cant just take a form from them.Walgreens does have them,but the customer service is crappy and they make you wait much longer than needed. When you do go to a pharmacy for your supplies,they need a brand new hand written prescription from your doctor to go with the form.
